SUPPORTIVE HOUSING - CBFS

LOCATION: 128 Park St, Dorchester MA

SCHEDULE: 40 hours, Monday-Friday 8:30am-5:00pm

JOB DESCRIPTION:

The Rehabilitation Specialist will be responsible for direct service, case management and all aspects of treatment planning, including comprehensive assessments, individualized action plans, and progress notes and all relevant documentation.

REQUIREMENTS:

• BA/BS  (Degree requirement can be waived with an additional year of experience)
• Valid driver’s license in good standing
• Certified in MAP, CPR, and First Aid or ability to complete certification within 90 days of employment
• Minimum of two (2) years direct care experience providing services to adults with mental illness
• Proficiency in word processing and other computer functions
• Demonstrated professional communication and organizational skills
• Ability to sit for long periods of time
• Ability to climb stairs
• Ability to use a phone, computer, and other office equipment
• Occasional lifting
• Ability to travel among program sites
